What is RPM?

RPM is a digital healthcare service that allows your medical team to track your vital signs from your home using smart, connected medical devices.

If you are diagnosed with a chronic or acute condition that necessitate ongoing monitoring and it is medically necessary, conditions such as hypertension, Heart Failure, COPD, Asthma, Diabetes, Obesity, Malnutrition

Cost and Coverage

RPM services are covered under Medicare Part B.  You may have a copay. Total out-of-pocket costs are determined by secondary insurance coverage and deductible status. Standard Part B 20% coinsurance may apply, depending on secondary insurance or dual Medicaid eligibility

Requirements

Schedule an Initiating Visit: You must have an in-person or telehealth visit with your doctor to formally establish your RPM program.

 

Give Consent: Your provider will explain how the device works and outline any potential monthly copays. Your verbal or written approval will be noted in your file.

 

Receive and Test the Device: The clinic will set up a device for you. Many modern RPM tools use cellular chips, meaning they work right out of the box without needing home Wi-Fi.

Who Can provide RPM?

Your healthcare provider can provide RPM as long as they are qualified health professionals such as Physicians, Physician Assistants, Nurse Practitioners and you meet the criteria and necessary medical conditions. 

You can continue the program for as long as your healthcare provider deems it medically reasonable and necesary to manage your condition. (HHS.GOV)

Your provider is able to extend RPM indefinitely to help prevent declines and hospitalizations.
